About the Role

CareGivers America, a Modivcare service, is a regional provider of Non-Medical Home Care and Private Duty Nursing services. Our team is dedicated to delivering high-quality care to our clients, and we're seeking a skilled nursing professional to join our team.

Responsibilities
  • Provide professional nursing care as defined in the PA Nurse Practice Act
  • Provide skilled nursing visits for clients as directed by the Assistant/Clinical Supervisor
  • Follow the care plan established by a Registered Nurse Case Manager
  • Recognize and report changes in client condition to the Assistant/Clinical Supervisor and physician, as directed
  • Communicate information on a timely basis, consistent with CareGivers America policy and urgency of the situation
Requirements
  • Graduate of an accredited school of professional nursing
  • Current license to practice as a Registered Nurse/Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment
  • Minimum of one (1) year experience in an acute care setting, home health experience preferred
  • Current CPR certification
  • Strong written, verbal, and interpersonal communication skills
  • Licensed driver with automobile insured in accordance with state and/or CareGivers America requirements and in good repair
