Overview
As a pivotal member of our Comfort Care team, the Registered Nurse (RN) specializing in Palliative Care delivers compassionate, patient-focused hospice services directly at the bedside of our participants. This role emphasizes the coordination of comprehensive physical, emotional, and spiritual support through collaboration with our interdisciplinary team.
Our Comfort Care RNs are dedicated to ensuring high-quality end-of-life (EOL) care that prioritizes symptom management, pain relief, and psychosocial support during transitions.
They provide professional triage, nursing care, and essential education to family members regarding the end-of-life process, symptom management, and emotional support during challenging times.
The RN serves as the primary contact for participants and their families throughout this significant transition.
Operating with a high degree of autonomy, our nursing professionals are expected to utilize critical thinking skills, assess family needs, and communicate effectively within the interdisciplinary team.
Participant Care- Assess and prioritize participant and staff phone inquiries to ensure appropriate care delivery.
- Execute orders from primary care physicians.
- Educate participants and families on medication management and treatment protocols.
- Administer and monitor medications, providing training on self-administration and adherence.
- Assist in the evaluation and treatment of participants, documenting and reporting on their conditions and needs.
- Conduct initial EOL assessments to identify individual participant needs and coordinate care plans.
- Develop and maintain EOL documentation, standards, and protocols relevant to the role.
- Perform initial assessments to determine participant needs and establish a coordinated care plan.
- Provide decision-makers with options and resources to facilitate informed choices in the participant's best interest.
- Educate families about the end-of-life process and distribute relevant caregiving resources.
- Maintain accurate medical records and fulfill agency reporting requirements promptly.
- Collaborate as a member of the Multi-disciplinary Team, participating in meetings and care planning.
- Engage in developing and presenting staff training sessions, collaborating with in-home service technicians as necessary.
- Offer psychosocial support to participants and families, including bereavement resources in partnership with the Social Work department.
- Address participants' and families' spiritual needs by connecting them with appropriate resources.
- Participate in interdisciplinary team efforts to formulate comprehensive Plans of Care for InnovAge PACE program participants.
- Additional responsibilities may be assigned as needed.
Qualifications
Required:
- Valid Registered Nurse License issued by the state.
- Current CPR certification or the ability to obtain it.
- Minimum of 2 years of healthcare experience, with a focus on geriatrics, including at least one year in EOL/Hospice care.
- Knowledge of supportive and bereavement counseling techniques and resources, with the ability to apply this knowledge effectively.
- Experience in conducting end-of-life assessments and developing suitable care plans.
- Personal transportation, a valid state-issued driver's license, a clean driving record, and auto insurance.
- Associate's degree in nursing.
Preferred:
- Bachelor's degree in nursing.
- Certification as a Gerontological Nurse.
- Over 3 years of healthcare experience with a focus on geriatrics and a broad understanding of healthcare challenges faced by the elderly.
- Bilingual proficiency in Spanish.
