BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to clients in their homes.
Job Responsibilities- Conduct home visits to assess clients' physical needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Implement evidence-based interventions, such as manual therapeutic exercises and gait training, to promote clients' independence and well-being.
- Educate clients and their families on rehabilitative care and activities to support their health and safety.
- Document observations, interventions, and evaluations to ensure accurate client care management.
- A current Physical Therapist license in HI state.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
