Job Summary

Compassus is seeking a skilled Hospice Social Worker to join our team. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and their families in need of hospice services.

Key Responsibilities
  • Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans to address these needs.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication with patients, families, and healthcare providers.
  • Provide emotional support and counseling to patients and their families, addressing concerns related to grief, loss, and end-of-life care.
  • Facilitate advance care planning, including living wills, power of attorney, and funeral arrangements, as needed.
  • Participate in care planning meetings and contribute to the development of comprehensive care plans that address patients' physical, emotional, and spiritual needs.
  • Document patient interactions, assessments, and care plans in a timely and accurate manner, adhering to Compassus' documentation standards.
  • Stay up-to-date with relevant laws, regulations, and industry standards, ensuring compliance with Compassus' policies and procedures.
Requirements
  • Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
  • Minimum one year of experience in a healthcare setting, preferably in hospice care or a related field.
  • Licensure as a social worker, as required by state regulations.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility to work in a fast-paced environment and adapt to changing priorities and circumstances.
