Job Summary:

The Licensed Practical Nurse will work in an ambulatory care setting, providing nursing care to patients in a variety of settings. This includes performing specialized procedures, administering medications, and educating patients and families on health-related topics. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Obtain patient history and gather information to provide comprehensive care.
  • Perform specialized procedures, examinations, and tests as ordered and aligned with the Ambulatory Clinical Scope Document.
  • Ensure all patient care areas are adequately stocked with unexpired supplies and medications.
  • Relay information and facilitate communication between the provider and the patient concerning patient care.
  • Evaluate patient's/caller's health status based upon presenting symptoms and follows defined protocols.
  • Tracks, reviews, and monitors the follow-up of diagnostic test results to ensure the physician/APP have verified results and patient has been notified.
  • Assists in the management of patient care between visits.
  • Completes necessary paperwork, prior authorizations, and/or scheduling of appointments for patients who are ordered diagnostic and therapeutic services.
  • Maintains patient records to ensure accurate documentation of the patient's medical treatment.
Requirements:
  • Diploma Program Certificate of completion (degree or diploma) from an accredited LPN program.
  • 1 year Clinical nursing experience.
  • Licensed Practical Nurse Upon Hire Required.
