Overview:

We are seeking a skilled and compassionate Home Care RN to join our team at UnityPoint Health. As a Home Care RN, you will provide high-quality patient care in the comfort of patients' homes, utilizing your clinical expertise and physical assessment skills to deliver exceptional patient outcomes.

Responsibilities:

  • Develop and implement individualized care plans for patients, ensuring seamless transitions between care settings.
  • Conduct thorough patient assessments, identifying needs and developing strategies to address them.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes.
  • Provide education and support to patients and their families, empowering them to manage their health and well-being.
  • Document patient progress and outcomes, utilizing data to inform care decisions and improve patient outcomes.

Qualifications:

  • Current RN licensure in the state(s) where providing care.
  • Minimum one year of experience in home health care or a related field.
  • Valid driver's license and automobile insurance.
  • Mandatory reporter certification.
  • Basic Life Support (BLS) certification.
  • Competency in nursing skills, including wound care, IV therapy, and catheter care.
