Job Summary

Social Worker - MSW or MSS - Acute Care

ChristianaCare is seeking a skilled Social Worker to join our team in an acute care setting. As a Social Worker, you will play a critical role in assisting patients and families to identify social, emotional, and environmental needs and connecting them with available resources and services.

Key Responsibilities:

  • Formulate a discharge plan that provides patients with services in the appropriate post-acute care setting.
  • Gather and assess information regarding patients' physical needs, mental status, family support systems, financial resources, and available community and governmental resources.
  • Develop a comprehensive case management plan that addresses patients' goals of care and identified needs for discharge planning.
  • Collaborate with other healthcare professionals in multi-disciplinary meetings and patient rounds.
  • Receive updates and guidance regarding Utilization Management from appropriate UM staff to assist with length of stay coordination.

Requirements:

  • Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school of Social Work.
  • Active social work license in the state of Maryland (in good standing).
  • Two years of social work experience in an acute care setting preferred.
