Job Summary

We are seeking a highly skilled and experienced Rehabilitation Team Lead to join our team at Consonus Healthcare. As a key member of our rehabilitation department, you will be responsible for leading a team of therapists and overseeing the delivery of high-quality patient care.

Key Responsibilities
  • Lead and manage a team of rehabilitation therapists to ensure seamless delivery of patient care
  • Develop and implement evidence-based rehabilitation programs to achieve optimal patient outcomes
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Manage financial goals and ensure regulatory compliance
  • Apply a transformative approach to therapy, leveraging clinical data to craft personalized care plans
Requirements
  • Graduate from an accredited education program with a degree in Physical Therapy, Occupational Therapy, Speech-Language Pathology, Physical Therapist Assistant, or Occupational Therapy Assistant
  • Current state licensure or in the process of obtaining appropriate state licensure
  • CPR Certification (requirement varies based on State/location)
  • National Provider Identifier (NPI) required upon hire
  • Experience in post-acute care preferred
