Job Summary:

As a Home Health Physical Therapist, you will play a vital role in providing high-quality patient care and rehabilitation services in the comfort of patients' homes. Your expertise will help patients regain independence, manage chronic conditions, and improve their overall quality of life.

Responsibilities:
  • Evaluate patients' physical abilities and develop personalized treatment plans to address their unique needs.
  • Conduct therapy sessions in patients' homes, using evidence-based techniques and technologies to promote optimal outcomes.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
  • Document patient progress, adjust treatment plans as needed, and communicate effectively with patients, families, and healthcare providers.
  • Supervise and mentor physical therapy assistants to ensure high-quality care and efficient workflow.
Requirements:
  • Bachelor's Degree or equivalent in Physical Therapy required; Master's Degree preferred.
  • Current Physical Therapy licensure in the state of California required.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
