Job Title: Physical Therapist

We are seeking a highly skilled and dedicated Physical Therapist to join our team at HexaQuEST Health, Inc. as a key member of our Rehabilitation Services department.

Job Summary:

The successful candidate will be responsible for providing high-quality physical therapy services to patients in various settings, including hospitals, clinics, and patient homes. This role requires strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional patient care.

Key Responsibilities:
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Provide individualized physical therapy services, including exercises, activities, and equipment training, to patients with a range of diagnoses and functional deficits.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and maintain accurate records in accordance with regulatory requirements.
  • Participate in quality improvement initiatives and contribute to the development of clinical programs and services.
Requirements:
  • Master's or Doctorate degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist and Basic Life Support certification.
  • Valid driver's license and ability to travel to remote locations as needed.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong clinical skills and knowledge of physical therapy principles and practices.
