Pediatric Care Provider

Job Summary:
We are seeking a highly skilled Pediatric Care Provider to join our team at UW Medical Foundation. As an Ambulatory Care Team Member, you will play a vital role in providing high-quality, cost-effective care to patients and their families in our outpatient primary care pediatric clinic.
Responsibilities:
- Participate as a care team member in managing a panel of patients in partnership with our primary care physicians.
- Evaluate and treat acute and chronic health conditions and promote wellness in our outpatient primary care pediatric clinic.
- Provide counseling and education about common pediatric illnesses and promote wellness with patients and families.
Requirements:
- Active DEA license or ability to apply for such license prior to or upon hire.
- Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.

Education and Qualifications:
- Minimum - If Nurse Practitioner: Master's or Doctorate's degree from accredited Nursing Program.
- If Physician Assistant: Bachelor's or Master's degree from accredited PA Program.
- State of Wisconsin NP or PA licensure.
- Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND Active board certification as a NP in area of clinical practice.
- Active PA license, including prescriptive authority, in state(s) of practice AND Active PA board certification.
