Overview:
We are in search of a dynamic and proficient case manager who possesses a solid understanding of IMM, MOON, and HINN notices, along with experience in MCG guidelines.

Facility Details:
This role is within a 16-bed Critical Access Hospital located in a serene environment, featuring a single Case Manager.

Patient Care Ratio:
The average patient ratio is less than 1:15, with a typical daily census of approximately 4.7 patients.

Housing Options:
There is a potential for shared accommodation in a 2-bedroom, 2-bath apartment. Please refer to the facility group documentation for further details. Note: Absolutely no pets are allowed.

Traveler Requirements:
The permanent address of the traveler must be at least 65 miles away from the facility. Local candidates will not be considered for this position.

Contractual Information:
Guaranteed Hours: 40 hours per week
Contract Duration: 91 weeks
