We are seeking a skilled Case Manager Flex to join our team at Northwestern Medicine. As a key member of our case management team, you will play a critical role in ensuring the optimal quality and efficiency of patient care and services.
Key Responsibilities- Assess patients within your defined geographical scope of service and determine case management needs.
- Participate in the multi-disciplinary team on all cases with identified needs for care coordination.
- Collaborate with the multi-disciplinary team to determine appropriateness of care and work to maximize care and services.
- Work closely with the RN staff and charge nurse to facilitate the patient through the care continuum.
- Assist hospital in meeting budgeted LOS by managing ADOD for assigned patients.
- Participate in LOS rounds by providing knowledgeable clinical specifics for patient information including discharge plan and barriers.
- Work closely with the multidisciplinary team to ensure patient flow through capacity management techniques.
- Trends care delivery related issues and provides information to the health Team.
- Communicates all regularly concerns to the Healthcare Team.
- Graduate of an accredited school of professional nursing.
- Three or more years of experience in acute care nursing, including inpatient and outpatient.
- Licensed to practice as a Registered Nurse in the state of Illinois.
- Bachelor of Science in Nursing.
- Case Management, Utilization Management, Discharge Planning, or related experience.
