Job Summary:

The successful candidate will provide high-quality, patient-centered care to our residents, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

Key Responsibilities:

  • Assess, plan, implement, and evaluate the nursing care for assigned residents, ensuring compliance with local, state, federal, and facility regulations and guidelines.
  • Communicate effectively with supervisors and physicians regarding changes in patient condition, responding quickly and accurately to changes in condition or response to treatment.
  • Perform initial and ongoing physical and psycho-social assessments, developing and implementing individualized care plans to meet the unique needs of each resident.
  • Administer prescribed medications, change dressings, clean wounds, and monitor vital signs, ensuring the highest level of patient safety and comfort.
  • Participate in treatment team conferences to assist in planning and revising goals, objectives, and interventions appropriate to the age-related and problem-specific needs of each patient.
  • Implement nursing plans of care for assigned patients, conducting and/or co-leading group therapy sessions as needed.
  • Evaluate patient responses to interventions, revising nursing plans of care as necessary to ensure optimal outcomes.
  • Collaborate with the treatment team to revise goals, objectives, and interventions in response to changes in patient status.
  • Monitor, record, and communicate patient condition as appropriate, utilizing computerized documentation systems to ensure accurate and timely information.
  • Instruct and educate patients and families, providing age and culturally appropriate care to meet the unique needs of each individual.
  • Follow Standard Precautions, utilizing personal protective equipment as required to ensure a safe and healthy environment for patients and staff.

Requirements:

  • Current FL RN License
  • Current BLS Certification as a Healthcare Provider (American Heart Association)
  • Previous work experience in a long-term healthcare setting preferred
