Job Summary

We are seeking a skilled Hospice Registered Nurse (RN) to join our team as a Full-Time Hospice RN/RN Case Manager. In this role, you will provide comprehensive end-of-life care to patients and their families, while also managing your caseload and collaborating with our interdisciplinary team.

Responsibilities
  • Assess and evaluate patient needs, developing personalized care plans to meet their physical, emotional, and spiritual needs.
  • Provide direct patient care services, including wound care, medication management, and symptom control.
  • Act as a liaison between healthcare providers, patients, and families, ensuring seamless communication and coordination of care.
  • Maintain accurate and timely documentation of patient information, including clinical records and progress notes.
  • Collaborate with our interdisciplinary team, including social workers, chaplains, and volunteers, to ensure comprehensive care.
Requirements
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
