Job Summary:

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team as a PRN Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments in the comfort of patients' homes, providing critical insights into their overall health and well-being.

Key Responsibilities:

• Conduct thorough health assessments in patients' homes, including medical history, medication adherence, and social environment.
• Develop and implement personalized care plans to address identified health risks and needs.
• Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
• Provide education and support to patients and their families to promote healthy behaviors and disease management.
• Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols.

Requirements:

• Current licensure as a Nurse Practitioner in the state of Pennsylvania.
• Valid driver's license and reliable transportation.
• Ability to travel to patients' homes and other community environments.
• Strong computer skills and familiarity with electronic health records software.
• Excellent communication and interpersonal skills.
