Job Summary

We are seeking a skilled Physical Therapist to join our team at Canton Potsdam Hospital. As a Physical Therapist, you will be responsible for completing patient assessments, establishing patient care plans, and administering patient treatment.

Key Responsibilities
  • Conduct thorough physical therapy evaluations to establish treatment plans and home programs
  • Administer patient treatment plans involving application of physical therapy techniques and modalities
  • Monitor patient response and progress, updating and modifying treatment plans as necessary
  • Instruct patients and family members/caregivers in physical therapy procedures and self-care techniques
  • Direct care provided by physical therapy assistants
  • Perform discharge planning and documentation
  • Consult with physicians and other healthcare professionals
Requirements
  • Master's Degree in Physical Therapy
  • Active and clear NYS PT license or temporary permit
  • BLS certification
  • Preferred: minimum of 1 year of post-licensure practice as a Physical Therapist
