Evening Hospice Registered Nurse

Good Shepherd Hospice is in search of a skilled Registered Nurse (RN) to provide compassionate care to patients and their families in the hospice setting.
The Hospice RN is responsible for conducting nursing assessments, planning, and delivering care to enhance the comfort and well-being of patients and families, in alignment with the Interdisciplinary Plan of Care.
- Work Schedule: Monday 5pm to Friday 8am
- Prior experience in hospice care is preferred
- Receives patient assignments from the Patient Care Manager or Case Manager, initiating communication with attending physicians, hospice staff, and other agencies as necessary to ensure optimal care and resource utilization for the patient and family.
- Develops communication and bereavement skills.
- Maintains consistent communication with the attending physician regarding patient and family care.
- Contacts families of newly assigned patients and maintains regular communication as requested or desired.
- Gathers information on physical, psychological, social, and spiritual factors that may affect patient and family health status, integrating this data into the Plan of Care.
- Keeps accurate and up-to-date patient records, ensuring that problems, plans, actions, and goals are clearly documented and changes are reflected promptly. Submits paperwork as required by Medicare and Good Shepherd Hospice policies.
- Instructs patients and caregivers on:
- Medication administration, including recognition of side effects and adverse reactions.
- Personal care and oral hygiene practices.
- Assistance with lifting, moving, ambulation, and exercise.
- Disaster and safety management.
- Preparation for events preceding death.
- Informs the Patient Care Manager of any unusual or potentially concerning patient or family issues.
- Participates in hospice team meetings and patient conferences, including care plan meetings, as deemed necessary by the Patient Care Manager.
- Provides appropriate support at the time of death.
- Shares in providing 24-hour, seven days a week coverage to patients and families.
- Evaluates hospice aides biweekly.
- Evaluates LPNs at least once a month.
- Engages in individualized studies to deepen understanding of hospice philosophy.
- Strives to become proficient in pain management through reading, in-services, or seminars.
- Completes all other assigned tasks.
Skills, Knowledge, and Expertise
- Registered Nurse currently licensed in the state where care is provided.
- Preferred experience in hospice care.
- High integrity, including the maintenance of confidentiality regarding all patient records.
- Ability to work collaboratively as a team player, following directions and adhering to established care plans.
- Demonstrated commitment to the hospice philosophy of care.
- Strong organizational and self-management skills.
- Critical thinking abilities with sound judgment.
- Successful completion of background checks.
- Valid Driver's License.
- Mileage reimbursement.
