Job Summary:
The Case Manager Registered Nurse is responsible for the overall development and management of the Cooperative Care Program. This role requires a strong clinical background and excellent communication skills to effectively manage patient care and ensure quality outcomes.
Key Responsibilities:
- Develop and implement cost-effective strategies to improve patient outcomes.
- Maintain knowledge of current and future trends in the healthcare environment, including payer policies and requirements.
- Collaborate with physicians, corporate leadership, and other stakeholders to ensure effective relationships and communication.
- Attend designated medical staff meetings to stay informed and provide updates.
- Ensure accuracy and integrity of all data gathered and analyzed in the department.
- Serve as a hospital liaison to third-party payers and facilitate compliance with regulatory agency standards.
- Identify opportunities for quality and utilization improvement in conjunction with physician advisor and medical director.
- Assist with budget preparation and compliance, including capital, expense, and FTE budgets.
- Participate in recruitment and retention efforts, including interviewing and hiring new staff.
- Provide leadership and support to ensure excellence within the department.
- Develop and implement staff and patient education programs related to CCP functions.
- Exhibit creativity and diplomacy in interactions with internal and external customers.
Requirements:
Education and Experience:
Active Kentucky RN license required. Minimum of five years clinical experience and five years of case management experience. CCM certification required. Previous management experience preferred.
