Key Responsibilities:- Round on patients as medically necessary, conducting H&P, admission, progress, transfer, discharge, and acute/follow-up assessments
- Work with the facility to transfer patients to the most appropriate level and site of care upon discharge
- Collaborate with hospice, palliative care, and home care agencies to ensure a safe transition home
- Productivity-based compensation model with uncapped income potential
- Estimated annual salary range of $85,000 to $105,000
