Job Overview

About the Role

The ideal candidate will have a Bachelor's degree and 2-4 years of related experience in healthcare management or a related field. A current state clinical license is preferred, but not required.

As a Care Navigator, you will be responsible for evaluating member needs, identifying barriers to care, and recommending and facilitating personalized care plans. You will work closely with healthcare providers, community resources, and other stakeholders to ensure seamless care coordination and optimal health outcomes.

Responsibilities
  • Evaluate member needs and identify barriers to care
  • Develop and implement personalized care plans
  • Collaborate with healthcare providers and community resources
  • Monitor progress towards care plan goals
  • Collect and maintain member information and care management activities
Requirements

To succeed in this role, you must have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. Strong analytical and problem-solving skills are essential, as well as the ability to manage multiple priorities and deadlines.
