Job Summary

As a skilled nursing professional, you will provide patient-centered care and case management to Home Care and Hospice patients in their homes. This PRN position offers a unique opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Administer skilled nursing care and case management in the home to Home Care and Hospice patients as authorized by the physician.
  • Develop, implement, and evaluate the nursing care plan for each patient.
  • Observe patients for signs and symptoms of complications and report to the physician.
  • Supervise home health aides and LPNs assigned to patients.
  • Promote health and well-being through teaching, supervising, counseling, and preventative measures.
  • Collaborate with other healthcare professionals to provide comprehensive care.
  • Take call on weekends, holidays, and evenings on a rotating basis and respond to patient needs as appropriate.
  • Comply with Livingston HealthCare policies and procedures.
  • Provide evidence-based care, education, and support for hospice patients, families, and caregivers.
  • Document care responsibly and accurately.
  • Track and record mileage on the appropriate form.
  • Participate in Quality Improvement initiatives.
Requirements
  • Current Montana RN license.
  • BSN, AD, or Diploma.
  • Good assessment skills.
  • Ability to function independently.
  • Good communication skills.
  • Ability to effectively function in a fast-paced environment with multiple interruptions.
  • Own vehicle for visits.
  • Current Motor Vehicle License.
  • Current BLS.
Preferred Qualifications
  • At least one year of recent nursing experience, preferably in community health and/or care of terminally ill.
