We are seeking a highly motivated and dedicated Innovation Coordinator to join our team at The Charles Lea Center. As an Innovation Coordinator, you will play a vital role in supporting individuals with disabilities to live, work, and play in their communities.
Job Summary:The Innovation Coordinator will work under the guidance of the Regional Manager of Innovative Services to provide services and supports focused on achieving goals for the people we support. This includes providing technical assistance to Community Providers, advocating on behalf of individuals and their guardians, and conducting follow-up assessments to ensure satisfaction with services.
Key Responsibilities:- Provide services and supports focused on achieving goals for the people we support.
- Provide technical assistance to Community Providers implementing services for people referred.
- Advocate on behalf of person/guardian to assure that services being provided reflect the desires of person/guardian.
- Conduct 30, 60, 90 day follow-up to assure satisfaction with services and those desired outcomes are being achieved.
- Follow the Standards, regulations required by Tennessee Department of Intellectual & Developmental Disabilities.
- At least 18 years of age.
- Ability to pass a background check upon hire.
- Valid driver's license.
- Bachelor's Degree in Human Services or a related field with two (2) years of experience in providing or coordinating services to people with disabilities and special needs and/or with case management services.
- Four (4) Years Case Management experience in providing or coordinating services to people with disabilities and special needs and/or with case management services.
The Charles Lea Center is a CQL accredited organization that provides services in South Carolina, North Carolina, and Tennessee. We are committed to providing innovative and person-centered services that support individuals with disabilities to live, work, and play in their communities.
