Job Summary

Clovis Healthcare and Rehab Center in Clovis, NM is seeking a skilled Wound Care Coordinator to lead our Skin Health and Wound Management program. As a key member of our nursing team, you will be responsible for developing and implementing a comprehensive wound care program that promotes patient-centered care and improves skin health outcomes.

Key Responsibilities
  • Lead and integrate a fully developed Skin Health and Wound management program in collaboration with nursing center management.
  • Oversee durable medical equipment related to the prevention and management of wounds.
  • Develop and implement wound care protocols and procedures to ensure compliance with regulatory requirements.
  • Collaborate with interdisciplinary teams to provide comprehensive wound care services.
  • Conduct wound assessments and develop individualized care plans to promote wound healing and prevent further complications.
  • Provide education and training to nursing staff on wound care best practices and protocols.
  • Monitor and evaluate wound care outcomes to identify areas for improvement and implement changes as needed.
Requirements
  • Must possess a RN license (MPT/DPT with leadership review/approval)
  • Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire
  • Commitment to attend monthly support calls is required
  • Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire
  • Knowledge and experience with clinical charting, incident reporting and investigation response is required
  • Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team
  • Experience in team building and demonstrating respect for the interprofessional team in wound management is required
  • Knowledge of nursing home regulations and survey process is required
  • Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required
