About the Role

We are seeking a skilled Occupational Therapist to join our team at AT Home Care & Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and supporting our patients in achieving their goals.

Responsibilities
  • Evaluate patient's functional status and home environment to identify equipment needs and develop a comprehensive plan of care.
  • Develop and implement treatment programs to improve patient function and independence.
  • Teach patients and their families new skills and retrain in daily activities to promote optimal care and outcomes.
  • Maintain accurate and timely clinical records and communicate effectively with other healthcare professionals.
  • Supervise Occupational Therapy Assistants and participate in discharge planning to ensure seamless transitions of care.
Requirements
  • Graduate of an occupational therapist education program accredited by ACOTE with current licensure to practice in the state of operation.
  • 2+ years of occupational therapy experience, with home health experience preferred.
  • Commitment to clinical and documentation excellence, with a focus on delivering high-quality patient care.
