Northwestern Lake Forest Hospital is committed to providing exceptional health care through state-of-the-art clinical services with compassionate and personal care. Diagnoses and treats speech, language, voice, and swallowing disorders.
Responsibilities
- Develops and implements physical therapy programs to maximize function and educates patients about their limitations.
- Evaluates patients and gathers information to determine accurate clinical diagnoses and treatment plans.
- Communicates treatment programs to patients, family, and interdisciplinary personnel; provides emotional support as needed.
- Coordinates and implements treatment plans, modifying based on patient progression.
- Treats patients using various techniques, including therapeutic exercise, functional activities, manual techniques, and modalities.
- Evaluates home, work, and community settings and makes recommendations and referrals for safe and efficient function.
- Re-evaluates patients at appropriate timeframes and documents goal progression to maintain effective plans of care.
- Plans and communicates discharge to patients, family, and medical personnel in a timely fashion.
- Maintains accurate and thorough documentation in accordance with department policy.
- Develops and presents educational programs for the department, organization, and community as needed.
- Demonstrates awareness of regulatory specifications, incorporates regulatory information into daily work, and is knowledgeable of Illinois State Practice Act.
- Adapts schedule based on clinic needs and encourages cooperation, collaboration, and co-ownership of processes.
- Contacts referral sources to promote positive relationships with Northwestern Medicine Lake Forest Hospital Rehab Services department.
- Actively participates in quality and process improvement efforts to increase quality of care and customer value.
