About the Role

Family Service League Inc. is seeking a dedicated and compassionate Social Worker to join our team at the Family Recovery Center in Mastic. As a Social Worker, you will play a vital role in providing assessments, direct care, and referrals to individuals struggling with mental illness, substance use disorder, or dual diagnosis.

Key Responsibilities
  • Manage a caseload of clients and provide case management services.
  • Conduct intake assessments and develop individualized treatment plans.
  • Provide individual and group therapy to adults, adolescents, and children with a range of mental health and substance use disorders.
  • Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record (EMR).
  • Follow up with clients during the stabilization period and immediately following.
  • Maintain up-to-date progress notes, treatment plans, and Outpatient Treatment Reviews (OTRs).
  • Coordinate with outside providers and collateral contacts.
  • Attend routine supervisions and team meetings.
  • Coordinate treatment with a multidisciplinary team.
  • Complete training and maintain proficiency with de-escalation and physical restraint skills.
Requirements
  • Master's degree in social work required.
  • LMSW, LMHC, LMFT, Limited Permit Social Worker or related licensed clinical degree required.
  • At least 1 year experience in a behavioral health setting required; clinic experience preferred.
  • Knowledge base of substance use disorders preferred.
  • Ability to problem solve, make clear decisions, and work as part of a team.
  • Excellent interpersonal, verbal, and written communication skills required.
  • Proficient computer skills, including Microsoft Office and Electronic Health Records required.
