Job Overview

We are seeking a highly skilled Patient Advocate to join our Kona Case Management Team.

">

Key Responsibilities:

">
  • ">
  • Conduct comprehensive assessments to identify consumer needs and develop personalized treatment plans.">
  • Collaborate with healthcare providers, insurance companies, and other stakeholders to ensure seamless service delivery.">
  • Provide ongoing support and case management services to consumers, focusing on prevention of re-hospitalization for psychiatric care.">
  • Participate in clinical supervision sessions to discuss consumer progress, ethical considerations, and cultural competency.">
  • Maintain accurate and timely clinical records, including progress notes and required forms.">
  • Perform other duties as assigned by management.">

    • Required Qualifications: ">
      • ">
      • Bachelor's degree in psychology, sociology, social work, or related human/social services field.">
      • One and a half (1 1/2) years of specialized training in case management or human services.">
      • Competency in providing services for dually diagnosed individuals is advantageous.">
      • Successful completion of a criminal background check and pre-employment drug screening.">
      • Possession of a valid Hawaii driver's license, vehicle insurance, and safety inspection.">
      • Current TB Clearance or TB X-Ray.">
      • Current CPR & First Aid Certificate.">

Estimated Salary: $50,000 - $65,000 per year
