Responsibilities:
  • Observe and record patient condition, notifying the care team and clinical supervisor of any changes or unusual conditions.
  • Assist with personal care, including hygiene regimens such as bathing, toileting, dressing, and grooming.
  • Support patients with household tasks, such as laundry, shopping, and meal preparation.
  • Educate patients on self-care techniques, including the use of canes, walkers, and equipment for personal hygiene.
  • Advise on nutrition and follow prescribed dietary requirements.
  • Follow the plan of care and assist with exercise.
  • Escort patients outside the facility and perform range of motion and other simple extensional therapy services.
  • Identify new recreational opportunities to help patients remain active.
  • Maintain strong working relationships with patients to build trust.
  • Ensure a safe and healthy patient environment by following security precautions.
Qualifications:
  • High School Diploma or GED.
  • CPR, First Aid, and Crisis Prevention certification.
  • At least one year of experience providing home health aide services.
  • Experience supporting individuals with disabilities, mental illnesses, or challenging behaviors.
  • Knowledge of basic medical skills and tasks, relevant trainings presented by NYSDOH.
  • Ability to access patients' vitals and properly document readings.
  • Ability to work with integrity, compassion, and attention to detail.
