About the Role

We are seeking a skilled Social Worker (BSW) to join our interdisciplinary team. As a key member of our hospice care team, you will provide vital psychosocial support to patients and their families, ensuring they receive the best possible care during life's most challenging moments.

Responsibilities
  • Conduct thorough assessments to identify patients' needs, goals, and interventions, aligning with our established plan of care.
  • Prioritize empathy and understanding when interacting with patients and their families, recognizing the complexities of terminal illness and loss.
Requirements
  • Bachelor's Degree in Social Work from a Council on Social Work Education (CSWE)-accredited institution.
  • Licensure as required by the state where our hospice is located.
  • A minimum of one year of experience as a Social Worker in a healthcare or hospice setting, with three to five years preferred.
