UPMC Family Hospice is seeking a dedicated Home Health Aide to provide patient-centered care to individuals with life-limiting illnesses in Allegheny, Beaver, and Butler Counties. As a Home Health Aide, you will work Monday to Friday, daylight shifts from 8:00 a.m. to 4:30 p.m., providing essential care and support to patients in their homes.


Responsibilities:
  • Provide patient-centered care, focusing on the physical, emotional, and social needs of patients.
  • Aid patients with bathing, dressing, and other activities of daily living, promoting independence and dignity.
  • Collaborate with healthcare teams to develop and implement personalized care plans, ensuring seamless transitions and continuity of care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure comprehensive care and support.
  • Participate in ongoing education and training to enhance skills and knowledge, staying up-to-date with best practices and industry developments.
  • Meet the physical and emotional demands of the role, working in a fast-paced environment with a high level of autonomy.

Requirements:
  • High School Diploma or equivalent required; one year of experience preferred.
  • Certified Nurse Aide (CNA) certification or equivalent preferred.
  • Minimum of 75 hours of Nurse Aide training required; completion of UPMC Home Healthcare Nurse Aide Training Course available upon hire.
  • Valid driver's license and reliable transportation required.
  • Act 33, Act 34, and OAPSA clearances required.
