We are seeking a highly motivated and compassionate Registered Nurse to join our team as a Peritoneal Dialysis Nurse. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and education to individuals with end-stage renal disease.
Key Responsibilities- Provide comprehensive patient care in an independent setting, including assessing, diagnosing, and implementing complete care plans.
- Empower patients by teaching them to administer their own dialysis and take control of their health.
- Lead, coach, and guide patients, support systems, and care partners to ensure successful treatment outcomes.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Conduct home visits and on-call shifts to provide ongoing support and education to patients.
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 12 months of RN experience.
- Current driver's license, reliable transportation, and vehicle insurance in state of residence.
- Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
- Basic computer skills and proficiency in MS Word and Outlook.
- Previous Chronic Hemodialysis, Acute Dialysis, Home Hemodialysis (HHD) or Peritoneal Dialysis (PD) nursing experience.
- Training/Teaching/Healthcare Education background.
- Previous case management experience.
- Familiarity with, or excitement to learn about telehealth technologies.
- Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN), Certified Peritoneal Dialysis Nurse (CPDN).
