Advanced Practice Clinician for Glycemic Management

Job Summary: We are seeking an Advanced Practice Clinician to join our team at Upstate University Hospital. As a Glycemic Control Specialist, you will provide expert-level clinical care to patients with diabetes, working closely with our healthcare team to improve quality of patient care.
The ideal candidate will have a Master's Degree as a Nurse Practitioner or Physician Assistant, current National Certification consistent with their clinical practice, and CPR-BLS Certification. Inpatient, Endocrine, or Glycemic management experience is preferred.
Responsibilities:
- Provide inpatient glycemic management of diabetic patients
- Assess and evaluate patients, ordering appropriate laboratory testing and diagnostic studies
- Arrange for referrals, testing, consultation, and therapeutic services
- Maintain outstanding relationships with families, referring physicians, and healthcare team members
