As a Caregiving Specialist - Home Health Therapist, your key responsibilities will include:

• Providing comprehensive assessments and developing personalized care plans for patients
• Delivering high-quality physical therapy services, including evaluations and interventions, as ordered by physicians
• Participating in IDG meetings and care conferences to ensure seamless communication and coordination of patient care
• Maintaining accurate records and documentation of patient interactions and outcomes

To succeed in this role, you must have:

• A current licensure as a physical therapist in the state of Michigan
• At least one year of experience as a physical therapist in the past five years
• Active CPR certification
• Reliable transportation with a valid state driver's license and car insurance
