About the Position

This full-time position involves providing primary community support and case management to mentally ill, homeless, and substance abuse clients. The successful candidate will work closely with the Migrant Education Program, conducting intake screenings, and assessing client needs to determine eligibility for case management services.

Key Responsibilities
  • Conduct thorough assessments of client needs and develop personalized service plans.
  • Provide advocacy and support to clients in accessing necessary services and resources.
  • Maintain accurate records of case management activities and progress.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Promote a positive team approach and maintain professional standards.
Requirements
  • A minimum of an Associate's degree in Behavioral Health or a related field.
  • Two years of relevant work experience in case management or a related field.
  • Demonstrated ability to form effective relationships with clients, team members, and community providers.
  • Bilingual in English and Spanish is required.
  • Eligibility for Agency Affiliated Counselor Registration from the State Department of Health.
