POSITION SUMMARY:
A Licensed Physical Therapist (PT) provides therapeutic services to patients intermittently in their residences. This is conducted following physician directives and care plans under the guidance of the Director of Clinical Services/Nursing Supervisor.
REQUIREMENTS:
Graduation from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
1. Must hold a current license in the state(s) of practice.
2. Preferred experience of two (2) years.
3. Alignment with the philosophy and objectives of this Agency.
4. Capability to demonstrate initiative and independent decision-making.
KEY RESPONSIBILITIES:
1. Comprehends and complies with established protocols and procedures.
2. Administers physician-recommended physical therapy.
3. Aims to enhance or reduce the impact of physical disabilities in patients.
4. Strives to return individuals to their highest possible level of functioning within their capabilities.
5. Collaborates with other home care staff in the development of patient care plans.
6. Oversees and directs staff as necessary.
7. Conducts initial assessments and evaluations.
8. Executes all skilled interventions as prescribed by the physician.
9. Consults with physicians regarding modifications in treatment plans.
10. Prepares reports for physicians detailing patient progress.
11. Educates patients and their families/significant others on home care routines and daily living activities.
12. Engages in and presents in-service training sessions as assigned.
13. Participates in Quality Assessment and Performance Improvement initiatives as required.
14. Attends scheduled patient care meetings.
15. Documents clinical and progress notes accurately.
16. Assists physicians in assessing functional levels.
17. Aids in the formulation and adjustment of care plans as needed.
18. Consults with family members and Agency staff.
19. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within Agency-defined timelines.
20. Utilizes ICD-10 codes appropriately.
WORK ENVIRONMENT:
Operates in Agency offices and patient residences, with travel to/from patient locations.
JOB RELATIONSHIPS:
1. Reports to: Director of Clinical Services/Nursing Supervisor
2. Supervises: Physical Therapy Assistants and Home Health Aides as assigned.
RISK FACTORS:
High risk exposure.
LIFTING REQUIREMENTS:
Ability to perform the following tasks as necessary:
· Engage in physical activities.
· Stand for extended periods while involved in physical tasks.
· Perform heavy lifting.
- Ability to engage in frequent bending, lifting, and standing.
