Intensive Foster Care Program Coordinator

Job Summary

The Intensive Foster Care Coordinator is a critical role within our organization, responsible for developing and monitoring plans of care that focus on the stabilization of youth enrolled in the Intensive Foster Care Program.

Key Responsibilities
  • Conduct comprehensive, strengths-based assessments of youth to identify their needs and develop individualized plans of care.
  • Collaborate with the Intensive Foster Care Family, resource parents, and other stakeholders to ensure the youth's needs are met and stability is maintained.
  • Monitor and evaluate the effectiveness of the plan of care, making adjustments as needed to ensure the youth's safety and well-being.
  • Provide education and consultation to resource parents and other providers on the values and principles of the Intensive Foster Care Program.
  • Ensure that all necessary data is gathered and recorded to evaluate the program's effectiveness.
Requirements
  • Bachelor's degree in social work or a related field.
  • Two years of experience working with youth and families in a social services or education setting.
  • Strong knowledge of and respect for youth and adults with mental health issues.
  • Excellent communication and writing skills.
  • Ability to work effectively with internal and external customers, including other professionals in the community.
  • Ability to work effectively as a member of a team.
  • Ability to manage time and work effectively with minimal supervision.
  • Ability to manage multiple priorities simultaneously.
