Physical Therapist

WeCare Centers is seeking a skilled Physical Therapist to join our team at Avalon Care Center in New Castle, PA.
Key Responsibilities- Develop and implement treatment plans to improve mobility and function in residents and patients with temporary or long-term disabilities.
- Deliver high-quality therapy services in accordance with established plans of care.
- Provide clinical support and education to patients, families, and caregivers.
- Monitor patient response to treatment interventions and adjust plans as needed.
- Maintain accurate and timely documentation of patient progress and therapy services.
- Valid state-issued license or certification as a Physical Therapist.
- Experience in skilled nursing environments preferred, but new graduates welcome.
- Excellent communication and instructional skills.
- Hands-on experience with therapeutic exercises, functional training, and physical modalities.
