Job Summary:

As a RN Case Management Travel Nurse, you will utilize your clinical expertise to assess and reassess patients, facilitate care coordination, and ensure medical appropriateness criteria are met. You will work closely with patients, families, and treatment teams to make informed decisions about next steps in care.

Responsibilities:
  • Review and analyze patient information to determine medical appropriateness and develop a plan of care.
  • Assess patient needs through observation, interview, and review of records, and work with treatment teams to develop a comprehensive plan of care.
  • Facilitate discharge planning and make referrals as needed to ensure a smooth transition of care.
  • Participate in performance improvement initiatives and contribute to the development of clinical effectiveness teams.
  • Communicate effectively with patients, families, and treatment teams to ensure a collaborative approach to care.
Requirements:
  • 1 year of nursing or case management experience.
  • Experience working with electronic medical records (EMR) preferred.
  • Working knowledge of regulatory requirements and accreditation standards preferred.
  • Ability to float to any Centura location within 60 miles of the original assignment location.
