About the Role

We are seeking a highly skilled Physical Therapist to join our dynamic home health care team in San Antonio. As a Physical Therapist, you will play a vital role in helping patients recover from illness or injury in the comfort of their own homes.

Key Responsibilities

  • Conduct thorough assessments to evaluate patients' mobility, coordination, strength, and current level of function.
  • Develop and implement effective treatment plans to achieve patients' goals and outcomes.
  • Provide high-quality physical therapy services to patients, ensuring compliance with prescribed physician orders.
  • Collaborate with healthcare teams, patients, and caregivers to ensure seamless care coordination.
  • Document patient progress, modify treatment plans as needed, and determine discharge care plans.

Requirements

  • Master's degree in Physical Therapy from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapist in the state of practice.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
  • Ability to drive to patients and establish a coverage area.
