Key Responsibilities:- Rounding and managing wound care for patients at SNFs
- Collaborating with wound care nursing staff to assess and treat patients
- Providing high-quality, patient-centered wound care services
- Timely documentation of patient care and encounters
- Availability for phone and secure messaging consultations with facility nursing staff
- Valid PA/NP license in the state of practice
- Valid NP/PA degree from an accredited institution
- Active DEA license (Theoria will assist with application for new graduates)
- General post-acute/SNF knowledge or experience preferred
- Familiarity with EMR systems and charting practices
- wRVU-based salary with monthly productivity bonuses
