Patient Care Coordinator RN

We are seeking a skilled Patient Care Coordinator RN to join our team. As a Registered Nurse, you will work closely with patients, families, and healthcare professionals to ensure seamless care delivery. Your primary focus will be on making the initial and/or comprehensive nursing evaluation visit, ensuring patients meet home health eligibility and medical necessity guidelines.
Main Responsibilities
- Coordinate total patient care with other healthcare professionals involved in care.
- Ensure continuity of patient care through planning and exchanging information with the physician, agency personnel, patient, family, and community resources.
- Make the initial and/or comprehensive nursing evaluation visit.
Requirements
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
