As a highly skilled Recovery and Reentry Case Manager at Cook Inlet Tribal Council, Inc., you will play a critical role in providing comprehensive case management services to participants involved in our Continuum of Care.

About the Role:

You will connect participants with essential resources, facilitate skills training, and coordinate care. Your exceptional communication and interpersonal skills will enable you to build strong relationships with partnering agencies and referral sources.

Key Responsibilities:
  • Create and manage clinical documentation for participants, including assessments, diagnoses, treatment plans, and progress notes.
  • Maintain participant records within the electronic health record and complete written reports, group/individual notes, assessments, etc., within the required documentation timeframe.
  • Maintain relationships with partnering agencies and referral sources, coordinate with community agencies and other treatment providers, and assist in the coordination of placement/transfer into treatment levels outside of assigned programs.
  • Conduct follow-up on referrals, communicate progress in case management goals and objectives to other treatment team members and participants, and observe and participate in group and individual sessions.

This is a full-time, regular position working primarily in an office setting with extended periods of time at a desk and on a computer. We offer a competitive salary of $**55,000** per year, commensurate with experience.
