Tufts Medicine Care at Home is seeking a skilled Home Health Case Manager to join our team. As a Home Health Case Manager, you will play a critical role in delivering high-quality, patient-centered care to individuals in their homes.
Key Responsibilities- Assess patients' physical, psychosocial needs in a sensitive, caring manner following established Standards of Nursing Practice and VNA procedures.
- Establish realistic goals and develop plans of treatment in cooperation with the patient, family, and members of the healthcare team.
- Collaborate with patient/family and other healthcare providers and/or community resources with planning of care and discharge.
- Complete physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies.
- Effectively manage assigned caseload, within the team model of care delivery.
- Promptly triage patient visits, messages, and phone calls according to priority and urgency.
- One (1) year of acute medical/surgical nursing experience.
- Excellent physical assessment and care planning skills.
- Current knowledge of pharmacology and medication administration and reconciliation.
- Ability to cope with patient/family emotional stress and provide appropriate supportive care.
- 403(b) retirement with company match.
