Job Summary:

As a Registered Nurse with Sevita, you will play a vital role in providing high-quality medical care to individuals with intellectual and developmental disabilities. You will be responsible for conducting medical assessments, developing and implementing service plans, and supervising the medical components of these plans. Your expertise will ensure that our clients receive the best possible care, and you will be an integral part of our mission to provide people with quality services and individualized supports that lead to growth and independence.

Key Responsibilities:
  • Conduct medical assessments and collaborate with intake staff to determine appropriate placement in the community
  • Participate in the development and implementation of service plans and supervise the medical components of these plans
  • Review documents and revise all nursing/nursing-related care plans on a regular basis
  • Maintain contact with primary care physicians and specialists, and attend doctor visits with clients as needed
  • Promote health and wellness for individuals and ensure nursing actions and steps are put in place for prevention
  • Administer medications according to established guidelines and provide documentation for all medications given
  • Oversee the management of high-risk medical conditions and private duty nursing services
  • Develop training flow charts specific to each client receiving services and provide medical skills training to clients, mentors, and staff
  • Participate in Individual Health Plan (IHP) or Plan of Care (POC) teams and evaluate the medical needs of each client for initial IHP/POC
  • Generate ongoing health care protocols for specific medical conditions outlined in the IHP/POC
  • Provide consultation and technical assistance to staff and mentors regarding home care issues and the operation of durable medical equipment
  • May be required to coordinate, organize, and/or assist with household activities such as light housekeeping and meal preparation, transport clients to planned and/or necessary activities and appointments, and participate in on-call systems as needed

Requirements:
  • Graduate of an accredited RN program
  • One year of experience working with medically complex individuals
  • Current state RN license
  • Current driver's license, car registration, and auto insurance if driving on behalf of the company or providing transportation to clients
  • Current CPR/First Aid certification

Physical Requirements:
  • Heavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects

Other Requirements:
  • Travels between multiple locations
