Responsibilities:
  • Assess patients' physical abilities and needs, and develop individualized therapy plans to improve mobility, strength, and overall functional ability.
  • Provide physical therapy treatments, including exercises, stretches, and other interventions to help patients recover from illness, injury, or surgery.
  • Collaborate with an interdisciplinary team, including physicians, nurses, and occupational therapists, to coordinate care and ensure optimal patient outcomes.
  • Educate patients and their families on therapeutic exercises, home programs, and strategies to maintain progress post-discharge.
  • Maintain accurate and timely documentation of patient assessments, treatments, and progress.
Requirements:
  • Current Physical Therapist (PT) license in good standing.
  • Minimum of 2 years of experience in a skilled nursing facility (SNF) or similar setting.
  • Strong knowledge of rehabilitation techniques, therapy equipment, and treatment planning in a long-term care environment.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with a multidisciplinary team and independently.
