Job Summary

The Ambulatory Care Nurse (ACN) plays a critical role in our organization, providing nursing care management services to assigned populations. As a key member of our interdisciplinary healthcare team, the ACN works collaboratively with physicians and other healthcare professionals to anticipate, integrate, and coordinate patient care plans, maximizing the ability of patients to remain in a community setting.

Key Responsibilities

  • Identify at-risk patient populations who require care management services
  • Collaborate with physicians to develop and implement patient care plans
  • Act as a patient/family advocate and communicate effectively with the healthcare team and community resources
  • Support the mission, values, and management of patient care services
  • Adhere to ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model

Requirements

  • 2 years of nursing experience or no experience required with preferred education
  • Diploma in nursing or associate's degree in nursing required; bachelor's degree in nursing preferred
  • Registered nurse licensure required

Work Schedule

8:00 AM - 4:30 PM, with occasional weekend and call requirements as needed.
