Job Summary:

As an Oncology Nurse Navigator at Hackensack Meridian Health, you will play a vital role in coordinating and navigating outpatient care for oncology patients. Your expertise and compassion will make a significant difference in the lives of our patients and their families.

Key Responsibilities:

Coordinate and navigate outpatient care for oncology patients
Identify new patients during the pre-diagnosis journey and offer navigation services from the beginning of the patient's diagnosis journey to survivorship or end of life
Build and maintain professional relationships with medical oncologists, radiation oncologists, surgeons, and other medical physicians and professionals involved in patient care
Establish communication with physicians, physician extenders, and their office staff to outline navigation programs and available resources
Collaborate with the primary medical oncologist, radiation oncologist, and/or oncology surgeon to act as a resource for the patient and family beginning during initial consultation and continuing through treatment
Maintain a communication system that ensures comprehensive and timely transmission of information to physicians, nursing staff, and the interdisciplinary team
Facilitate access to services, track the flow of patients through diagnostic and treatment services, and document assessment and follow-up notes in the patient's medical record via SMART PHRASE (as applicable)
Provide ongoing assessment of patient needs, including educational, financial, psychosocial, nutritional, and clinical needs, and connect patients and families to available resources within the health system and in the community
Work collaboratively with the research department and participate in the Tumor Board as needed
Develop and participate in performance improvement activities and develop guidelines in collaboration with Medical Directors and physician leaders to improve patient outcomes, assure appropriate utilization of resources, and increase patient satisfaction

Requirements:

Bachelor of Science in Nursing (BSN) (Note: this applies to team members hired on or after March 2024)
Minimum of two years of experience in nursing
NJ State Professional Registered Nurse License
Excellent written and verbal communication skills
Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms
Oncology experience preferred but not required
Oncology Certified Nurse preferred
Oncology or BMT experience preferred
