About the Role

Community Health Programs is a network of health centers and caring professionals that provide outstanding primary and preventive care for patients of all ages. Our team is dedicated to delivering high-quality healthcare services, and we are seeking a skilled Licensed Practical Nurse to join our team.

The LPN will be responsible for triaging patients by telephone or walk-in, determining the urgency and level of care needed. They will also assist physicians in procedures requiring nursing level support, administer immunizations, and manage requests for prescription medication. Additionally, the LPN will assist with running daily controls on equipment, charting for quality assurance, and ensuring that billing codes are correct.

Key Responsibilities

  • Triage patients by telephone or walk-in, determining the urgency and level of care needed.
  • Assist physicians in procedures requiring nursing level support, including administering immunizations and managing requests for prescription medication.
  • Assist with running daily controls on equipment, charting for quality assurance, and ensuring that billing codes are correct.


Requirements

  • Graduation from an accredited school of practical nursing.
  • A minimum of three years' experience in an ambulatory setting, preferably with electronic medical records experience.
  • CPR certification is a plus.
