Job Summary

We are seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with Elara Caring, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals and improve their overall health and well-being.

Responsibilities

  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs and goals.
  • Provide one-on-one physical therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and recovery.
  • Collaborate with other healthcare professionals, including nurses and social workers, to ensure comprehensive care and support for patients.
  • Communicate effectively with patients and their families to educate them about their condition, treatment options, and self-care strategies.
  • Document patient progress and outcomes in a timely and accurate manner, using electronic medical records.

Requirements

  • Current Physical Therapist license in good standing.
  • CPR certification with American Heart Association or American Red Cross.
  • At least one year of experience in a healthcare setting, preferably in home health care.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for long periods.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
